If you represent a small business applying for coverage for your employees, the business must first qualify and then employees at the qualified business must apply individually to participate.
Step by Step:
- Those interested in applying for CoverTN must complete the on-line qualification form and return the signed application to the State of Tennessee. The on-line form is only certified to work with Internet Explorer 5.5, Netscape 8 or higher browsers. Adobe Acrobat Reader is required to view and print your form once you have completed it and you must have your web browser set to allow for pop-ups. You may also call 1-866-CoverTN to request that an application be mailed to you.

- The State verifies eligibility, notifies individuals that they are qualified to participate in CoverTN, and authorizes BlueCross BlueShield of Tennessee to send an enrollment packet that includes a Participation Agreement and enrollment applications.
- Applicant mails a packet to BlueCross BlueShield of Tennessee that includes the completed Participation Agreement and applications.
- BlueCross BlueShield of Tennessee enrolls individual and begins debiting the bank account for future monthly premiums.
