Applying for CoverTN

Please review the Eligibility Information below to ensure that you qualify before attempting to complete the on-line qualification form. If you experience any difficulty in using the qualification form, please contact us.

If you represent a small business applying for coverage for your employees, the business must first qualify and then employees at the qualified business must apply individually to participate.

Step by Step:
  1. Those interested in applying for CoverTN must complete the on-line qualification form and return the signed application to the State of Tennessee. The on-line form is only certified to work with Internet Explorer 5.5, Netscape 8 or higher browsers. Adobe Acrobat Reader is required to view and print your form once you have completed it and you must have your web browser set to allow for pop-ups. You may also call 1-866-CoverTN to request that an application be mailed to you.


  2. Completed forms may be sent by fax to 615.253.8556 or mail to:

  3. The State verifies eligibility, notifies individuals that they are qualified to participate in CoverTN, and authorizes BlueCross BlueShield of Tennessee to send an enrollment packet that includes a Participation Agreement and enrollment applications.


  4. Applicant mails a packet to BlueCross BlueShield of Tennessee that includes the completed Participation Agreement and applications.


  5. BlueCross BlueShield of Tennessee enrolls individual and begins debiting the bank account for future monthly premiums.
